We offer a range of services for adults in the Aneurin Bevan Health Board area who would like support with losing weight. These services will provide you with the knowledge, tools and support you need for successful, sustainable weight loss with a focus on promoting health and wellbeing.
During the pandemic some of our services will be delivered remotely by telephone or video link. More details about the services provided and how you can refer yourself can be found below.
Children under 16 years can be referred to 'Connect' Child and Young Person Weight Management Service.
You can refer yourself for support to lose weight. You can do this by either filling out an online referral form or by telephone, where a member of our team will complete the form with you.
The easiest way to refer yourself to the Adult Weight Management Service or Foodwise is to fill in our online referral form. You will need to know your height, current weight and have a list of your current medication. The form should only take 5-10 minutes to complete. On the last page you will need to click ‘submit’.
The fastest way to refer yourself is by using our online form, but if you are unable to do that, then you can refer yourself by telephone. The call will take around 20-30 minutes and a member of our team will ask you questions about you and your weight. You will need to know your height, current weight and have a list of your current medication. Please call us on 0300 303 4906 on Tuesdays or Thursdays between 09:00am and 12:30pm to refer by telephone.
Our service is getting back to normal, however for now we are only providing our support with video calls, virtual groups and telephone calls. We are not offering face-to-face appointments for your safety and ours. Our team includes Dietitians, Dietetic Support Workers, Clinical Psychologists, Counsellors and Doctors. We run a range of groups and individual appointments to support people along their weight loss journey. Our level 3 MDT (Multi-disciplinary team) service has now re-started.
